If an injury/incident occurs, League safety officers now have two options to report the injury/incident information to the district:
1. Safety officer enters injury information through a Google form (
https://forms.gle/mixFHhmkmKzhxr41A) (no personal information). The completed form is sent to the District safety officer and the information exported to a password-protected spreadsheet.

OR
2. Send a copy of the ASAP Incident/Injury Tracking Report to the district safety officer:
[email protected]. The injury information will be recorded in the password-protected spreadsheet and all paper and electronic reports will be destroyed per HIPPA requirements.
The first option enables you to provide the injury data to the district without any accompanying identification for the player or family (no name, address DOB, etc). The second would send the reporting form in its entirety to me, and after recording the injury data, I will destroy the form per HIPPA requirements. Please check with your league if there is a preference for which option it will use.
We plan to use this information for several purposes, all with the goal of enhancing player and league safety, including but not limited to: tracking injury trends that would indicate trainings are needed to promote injury prevention, provide documentation for Little League purposes, to identify safety issues at fields that need to be remedied, provide injury reports by age that will help structure next year's safety clinic to age-appropriate material, etc.
