Each team is required to have a pitching affidavit on the back side of the tournament of champions affidavit.
REQUIRED FOR TOURNAMENT
Team Roster (must be regular season team)*
Signed by League President
Each manager and coach should have completed their three safety trainings
Each manager should have medical release forms for each player
*Note: Tournament of Champions is a District run tournament. The tournament teams is comprised of the regular season (Spring) team as rostered during drafts of the current year. No players can be pulled and/or moved to this team for purposes of competing in the tournament.
